Payment & Insurance

We provide equal access to all patients regardless of their payment source, but we do expect payment at time of service in form of cash, check or Visa/Mastercard. We accept all insurances listed. Please call your insurance company prior to your appointment and supply them with our tax ID# 23-1739482 to verify that we are a participating provider and select our office as your designated PCP. There have been instances where the insurance company does participate but your individual plan does not.

If your insurance company is not listed, please call them and supply our tax ID# 23-1739482 to see if your insurance may be covered under a parent company with which we are participating.

We are able to submit most secondary insurances for Medicare patients. However, if the secondary insurance is an HMO product or a Medical Assistance program with which we do not participate, we cannot submit for you. If you have difficulty, we will attempt to assist you in submitting the claim.
